General Instructions:

If you are one of the approximately 374,485 patients' of The Regents of the University of California dba UC San Diego Health (“Defendant”) whose personal information or personal health information was known to have been impacted by, or who received notice in September 2021 in connection with, a data incident that occurred between December 2, 2020 and April 8, 2021, whereby unauthorized parties accessed certain employee email accounts at the University of San Diego Health that contained patients personally identifiable information and protected health information, including full names, addresses, dates of birth, email addresses, fax numbers, claims information, laboratory results, medical diagnosis and conditions, Medical Record Numbers and other medical identifiers, prescription information, treatment information, medical information, Social Security numbers, government identification numbers, payment card numbers and financial account numbers and security codes, student ID numbers, and usernames and passwords, then you are a Settlement Class Member and entitled to participate in the Settlement.

If you received a notice about this class action Settlement addressed to you, then the Settlement Administrator has already determined that you are a Settlement Class Member. Available Benefits

As a Settlement Class Member, you are eligible to receive:

1. Two years of free medical and credit monitoring;
2. An approximate monetary cash payment amount between $169.69 and $33.94, if the participation rate is between 2% and 10%, respectively, before deducting the Documented Time and Out-of-Pocket Losses from the remaining Net Settlement Fund (“Monetary Payment”);
3. A cash payment of up to $175 for up to seven hours of documented time spent fairly traceable to the Data Security Incident valued at $25 per hour (“Documented Time”); and
4. A cash payment for documented losses and/or expenditures fairly traceable to the Data Security Incident (“Out-of-Pocket Losses”) subject to an aggregate claims cap of ten thousand United States Dollars ($10,000).

The medical and credit monitoring provided is Medical Shield Complete and valued at $14.95 per month. If you are already subscribed to Medical Shield Complete or a similar service by Pango Group, two additional years will be added to your current plan for free.

The Monetary Payment is for the alleged damages you suffered as a result of having your information allegedly disclosed during the Data Security Incident. The payment amount will depend on the participation rate for the Settlement and the amount will be each Settlement Class Member's pro rata share of the remaining Net Settlement Fund, after all other Settlement Benefits have been paid for out of the Remaining Net Settlement Fund, including payment for all claims for Out- of-Pocket Losses Payments and Documented Time Payments. Cash payments amounts may be reduced pro rata (proportionately) depending on how many people submit such claims.

If you are unable to locate your ID & PIN, please contact us by emailing info@UCSDHealthBreachSettlement.com. Please request your Unique ID & PIN by providing the Class Member's full name and mailing address.

If you did not receive direct notice of this Settlement and you are not among the 367,270 Settlement Class members for which Defendant has names and mailing addresses, then you must mail the paper claim form to the Settlement Administrator at the address below. Please follow all instructions outlined on the Claim Form and mail your completed Claim Form, including any supporting documentation, to:
